Following the deaths of 12 children in 2007 after their families had been visited by state social workers, Colorado has launched a review to determine whether systemic failures contributed to the deaths.

The Colorado Department of Human Services will send 21 investigators out to complete reviews in the 11 cases where comprehensive investigations have not been finished.

“Child fatalities occurring in Colorado over the past year lead me to believe that a comprehensive review of how we assess and address risk to children is necessary,” said Colorado Human Services Executive Director Karen Beye.

The review will look at all state rules on child welfare enacted in 2006 and 2007 and how they were applied in these cases, and whether there were common elements in the deaths.

No timetable was immediately released for the review.
