DENVER—The Colorado attorney general says the state is receiving about $1.2 million in a settlement with a dental management company serving low-income kids.
The Justice Department said Wednesday that FORBA Holdings LLC will pay $24 million in a multistate settlement of allegations that FORBA provided medically unnecessary and substandard services.
FORBA, of Nashville, Tenn., is associated with 69 “Small Smiles” dental centers nationwide.
