DENVER—Colorado labor officials are scrambling to reassure nearly 2,000 people who received tax forms printed with the wrong name.
The state says a computer glitch caused it to send erroneous 1099G forms that listed the wrong names but correct Social Security numbers. The error alarmed recipients but caused no real threat of identity theft because only the names were misprinted, not other information such as Social Security numbers.
The 1099G forms are for people who received unemployment benefits last year. The Colorado Department of Labor and Employment said a record-high number of people on unemployment last year contributed to the error. The state sent more than 100,000 of the forms.
Corrected forms were mailed Wednesday.
