• Because of a reporting error, a March 2 story on Page 1B, about a mentally disabled woman whose attorney has filed a notice to sue the University of Colorado dental school alleging 13 of the woman’s 14 teeth were pulled without her or her caregiver’s consent, incorrectly stated that a consent form stamped by the disabled woman did not mention extraction. The word “extraction” is handwritten on a form.
