• Because of incorrect information from a source, a story on Page 8A in Wednesday’s paper reported that Jim Richey, Lakewood’s first mayor, had failed three times to incorporate the city. In fact, the failed attempts were made by others.
• Because of a reporter’s error, a story on Page 1A on Wednesday said three Adams County deputies were disciplined in two 2009 jail incidents. In fact, they were not disciplined.
• Because of a reporter’s error, a story on Page 2A on Tuesday had incorrect information about Medicaid enrollment during the first two weeks of open enrollment. Medicaid enrollments in Colorado totaled 12,533 during that time. Also, an e-mail address for consumer questions should have been help@sc.connectforhealthco.com.
• Because of a reporting error, a story on Page 1A on Wednesday misidentified Larry Price, Lockheed Martin’s deputy director on the Orion mission.
