The Colorado Division of Insurance fined Cigna Healthcare-Centennial State Inc. $32,000 after a market conduct examination found violations of Colorado’s laws on health maintenance organizations.

After Cigna notified the state it had stopped marketing its HMO products in Colorado in August 2008, an audit found instances that the company failed to allow 30 days to provide additional information before denying claims and failure to have written denials of adverse determinations signed by a physician.
