DENVER—The Colorado Department of Human Services is making changes following a grand jury report critical of the staff at Colorado Mental Health Institute at Pueblo over patient care.

The grand jury found no criminal wrongdoing, but it issued a scathing report about problems in training, staffing and procedures at the state mental hospital.

The grand jury was convened to investigate the death of Troy Geske, who suffocated while staff restrained him in a prone position, and three other deaths following reports by KMGH-TV.
