DENVER—A state agency has lost thousands of applicant names on a computer disk.
The Colorado Department of Health Care Policy and Financing was required by federal law to notify the public after the names of nearly 3,600 medical-aid applicants were lost.
The disk did not include dates of birth, Social Security numbers or other personal information that could lead to identity-theft.
According to the Denver Post, the disk was lost on its way between two state agencies and it was discovered May 6. The people whose information was lost will get notices in the mail.
