PUEBLO, Colo.—The Colorado Mental Health Institute is under review after a patient attempted suicide and another suffocated after he was restrained by staff.

KMGH-TV in Denver reports that the death certificate for 41-year-old Troy Geske shows he died of positional asphyxia. That means he couldn’t breathe because of the way he was restrained. Pueblo County Coroner James Kramer ruled his death accidental.

The news station reports that Geske was restrained because the hospital staff said he was belligerent and out of control. After Geske was restrained, a staff member outside his room noticed he wasn’t moving.

Another patient at the hospital attempted suicide Sept. 20.

The state human services department has announced an independent review.
