A driver who died in an Adams County crash when her car was rear-ended by a van and slammed into a semi-trailer, has been identified.
Amy Candelarie, 45, of Fort Lupton, was taken to Denver Health Medical Center on Monday where she was pronounced dead, said Trooper Nate Reid, a Colorado State Patrol spokesman.
The crash happened at about 10:30 a.m. Monday on I-76 just east of Dahlia Street, Reid said.
A semi-truck with a trailer had come to a stop on the highway near a construction zone in which two lanes were pared down to one, Reid said.
Candelarie, driving a Honda Accord, came to a stop behind the semi when a van plowed into the rear of her car.
The driver of the van, Jason Sheffield, 34, of Thornton, was taken to Denver Health with moderate injuries, Reid said. A passenger in the Honda was also taken to the hospital with serious injuries.
The driver of the semi truck was not injured.
The crash remains under investigation.
